    Competitors will find this simple explanation helpful in reading the score sheets posted for each class.
  • Group Profile Form (PDF Fillable Form)
    This form is for USDAA affiliated groups to submit information about their group for presentation on the USDAA web site. Download the file, fill in the form and return via email to info@usdaa.com.
  • IRS Form W-9 - Request for Taxpayer Identification Number
    This IRS form is used to request a taxpayer identification number for purposes of reporting nonemployee compensation. A group may need this form for payment to judges, seminar instructors and others to whom they make payments for services. Backup withholding and special a special filing may be required. Consult your tax advisor or the IRS web site at www.irs.gov.
  • Group Licensing Agreement & Affiliation Requirements
    USDAA's Group Affiliation & Licensing Agreement and related information for conduct of a sanctioned match for groups seeking to become a USDAA "Licensed Group". Original multi-copy forms should be requested from the USDAA office at info@usdaa.com in order to begin the application process.
  • Sanctioned Match Application
    Submit this form along with the Group Application.
  • Application for USDAA Sanctioned Event
    UPDATED!   Form for licensed groups to apply for sanction of an agility test or event.
  • Judges Panel & Class Schedule Form
    This form is for groups to file their class schedule and judges panel for their sanctioned event. It may be filed with the Event Application form or subsequently.
  • Organizing Committee Report
    Groups use this form to report on a sanctioned event following conclusion of the event.
  • Four Year Event Planning Calendar
    Groups should use this calendar in determining event dates for future years. NOTE THAT THIS FILE WILL BE UPDATED FOR 2005 TO REFLECT REASSIGNMENT OF WEEKEND #'S AS PART OF THE IMPLEMENTATION OF POLICY ON ESTABLISHING EVENT DATES.
  • Dog/JH Registration Form
    If you cannot register online, then complete and mail in this form to register your dog for competition or a Junior Handler & Dog team for participation in the Junior Handler Program.
  • Application for Sanctioned JHP Test
    Licensing application for JHP groups to receive sanction of JHP test. Includes Class Schedule & Judges Panel form.
  • JHP Organizing Committee Report
    Groups use this reporting form following conclusion of a Junior Handler Program Only event.
  • Junior Handler Examination
    Junior handler's examination, 14 sample questions and answer key, to be used at events where Junior Handler Program classes are offered. Examination of 10 true/false questions is required to be administered to beginner level entrants.
  • Injury Reporting Form
    Use this form to report injuries to dogs, whether in competition, training or arising from other activities with possible links to performance in dog agility.

  • Course Reviewers' Region Map
    Effective for events beginning on or after October 1, 2006.  Offshore locations are part of the nearest mainland region. If there are any questions, contact the USDAA office at info@usdaa.com.

    This is the supervising judge's reporting form to be filed following a supervision assignment.
