2008 Grand Prix of Dog Agility® & Performance National Standard Rules Qualifiers held at shows that do not offer all levels of titling, will not include 'bye' vouchers as part of the awards. Only first place qualifying competitor at Regionals may receive the bye into the finals for world championships.
2008 Dog Agility Steeplechase® Rules Qualification criteria tightened for Round 2 at Regionals and for the Semifinals at the World Championships. Based upon detailed analysis of regional qualifications, 15% rather than 25% will be used for 2008.

IRS Form W-9 - Request for Taxpayer Identification Number This IRS form is used to request a taxpayer identification number for purposes of reporting nonemployee compensation. A group may need this form for payment to judges, seminar instructors and others to whom they make payments for services. Backup withholding and special a special filing may be required. Consult your tax advisor or the IRS web site at www.irs.gov.
Group Licensing Agreement & Affiliation Requirements USDAA's Group Affiliation & Licensing Agreement and related information for conduct of a sanctioned match for groups seeking to become a USDAA "Licensed Group". Judges Panel & Class Schedule Form This form is for groups to file their class schedule and judges panel for their sanctioned event. It may be filed with the Event Application form or subsequently.

Junior Handler Examination Junior handler's examination, 14 sample questions and answer key, to be used at events where Junior Handler Program classes are offered. Examination of 10 true/false questions is required to be administered to beginner level entrants.

Statement of Policy on Establishing Event Dates This statement sets forth the requirements for date selection, designation of "primary event dates" for groups, and sets forth radius limits for date selection when other USDAA events are held on the same weekend. This statement was ratified by the Board on November 11, 2004 and is effectively immediately, except for events already approved.
